Halloween Registration/Membership Drive

Enter through the doors on Center Street and look for the signs.

Registration Requirements:

  • Completed forms:Download Trick-or-Treat form here.Download Membership form here.If you do not participate in Halloween, but want to be a TETNA member, fill out the membership portion of the form only.(The form is in PDF format. You may fill out the form within Acrobat Reader or print it out and then fill it in. To save paper, we suggest you print one side; then the other on the same piece of paper.)
  • A LARGE, factory sealed bag of candy for each child being registered — please pick quality candy with at least 50 pieces per bag. Example: 2 kids = 100 pieces of candy; 4 kids=200 pieces of candy, etc.
  • $2.00 Per Child for TETNA Members (Cash or Checks Payable to TETNA)or $6.00 Per Child for Non-TETNA Members (Cash or Checks Payable to TETNA)

Please bring an item to donate for TOSA Cares – canned goods, dry foods, items for cold weather or personal care.
