Halloween Registration

Bethany Calvary Church, 7265 W. Center StreetEnter through the doors on Center Street and look for the signs.

Registration Requirements:

• Completed form (The form is in PDF format. You may fill out the form within Acrobat Reader or print it out and then fill it in. The form is two-sided. To save paper, we suggest you print one side; then the other on the same piece of paper.)
• A LARGE, factory sealed bag of candy for each child being registered — please pick quality candy with at least 50 pieces per bag.  Example: 2 kids = 100 pieces of candy; 4 kids=200 pieces of candy, etc.
• $2.00 Per Child for TETNA Members (Cash or Checks Payable to TETNA)
• $6.00 Per Child for Non-TETNA Members (Cash or Checks Payable to TETNA)

Please bring an item to donate for TOSA for Kids – canned goods, dry foods, items for cold weather or personal care.
